EGFR Mutation Analysis BCBC updates to Medical Policy |

Tennessee BCBS Medical Policy Manual
Effective 11/12/11 BCBS of Tennessee considers EGFR mutation analysis medically appropriate with ALL of the following criteria:
- Non-small cell lung cancer not associated with squamous cell type
- Testing is used to predict treatment response to tyrosine kinase inhibitors (erlotinib, gefitinib)
- Analysis of Exon 19 small deletions or Exon 21 point mutation (L858R)
Analysis for mutations including other mutations within exons 18-24, or other applications related to NSCL, is considered investigational.
North Carolina BCBC Medical Policy Manual
Effective 8/2011 BCBS of North Carolina may consider EGFR mutation analysis medically necessary when:
- Analysis of Exon 19 small deletions or Exon 21 point mutation (L858R)
- Testing is used to predict treatment response to erlotinib in NSCLC
Analysis of Exon 19 deletiona and Exon 21 (L858R) is considered investigational for patients with advanced squamous type NSCLC. Analysis for other mutations within exons 18-24, or other applications related to NSCLC, is considered investigational.

BCBST Has New Mailing Address |

Effective immediately, BlueCross BlueShield of Tennessee (BCBST) has a new claims mailing address and will no longer use a PO Box number. The new claims address for BCBST is:
BlueCross BlueShield of Tennessee 1 Cameron Hill Circle, Suite 0002 Chattanooga, TN 37402-0002
Please Note: The new address has been updated in MPLN's Symphony WebOrder system, so please choose this new address. Also, although the back of your patients' insurance cards may still indicate the old PO Box address, it is not correct. Any mail sent to the former BCBST PO Box address will be returned to sender. |

Insurance & Third Party Billing
For MPLN to bill an insurance company or third party administrator, be sure to include the following information on the requisition form:
- Patient's insurance information
- All applicable diagnostic codes in ICD-9 format
Patients are responsible for co-payments, deductibles and/or outstanding balances not covered by insurance. If your patient has a question about their laboratory bill, they are welcome to call MPLN directly.
Patient Billing
We can bill patients directly if the test requisition form is complete and includes the responsible party's name, address and telephone number. Patients can pay their bill online from MPLN by clicking here.
Client Billing
Our facility fee schedule may be discounted based on a client’s total test volume with MPLN and its subsidiaries. Clients are billed on a monthly basis by an itemized invoice, terms are payable upon receipt. Any adjustments are shown on the following month’s statement. Clients who have collections past due 90 days are sent a written notification.

This secure payment method allows patients to pay their bill from MPLN online at their convenience. Visa, Mastercard and Discover are all accepted.
To obtain record of payment, there is an online option to request an e-mailed receipt. If you have any questions, please contact the billing department at 800.600.1274.
