Notice of Cyber Incident

On December 17, 2021, Molecular Pathology Laboratory Network, Inc. (MPLN) experienced a cyber-incident that impacted some protected health information (PHI) within its data network. MPLN immediately shut down its data system, conducted an extensive investigation with the assistance of a third-party forensic firm, notified law enforcement, and implemented additional security measures. As part of MPLN’s initial investigation, it was discovered that a limited amount of information may have been accessed in connection with this incident. MPLN continues to work with outside forensics to determine the types of information impacted and to whom the information relates.

While MPLN’s investigation remains ongoing, the impacted information may have included one or more of the following data elements: name, address, date of birth, sex, phone number, email address, diagnosis/treatment information, procedure type, provider name, prescription information, date of service, medical record number, patient account number, procedure code, and medical test results.

The privacy and security of the personal information MPLN maintains on behalf of its clients is of the utmost importance. MPLN has established a dedicated assistance line for impacted individuals with questions or concerns at 855-919-5277, Monday through Friday, (except U.S. holidays), from 9 a.m. – 9 p.m., EST, or may be contacted by mail at 250 E. MPLN recommends individuals take actions to help protect their information by remaining vigilant in reviewing their account and explanation of benefits statements and consider placing a fraud alert and/or security freeze on their accounts.
